This form is used for submitting your signed ABDSM rules and guidelines and for completing your ABDSM profile which will be publicly displayed in the Find-an-ABDSM-Diplomate directory.

Your directory listing on ABDSM.org will be available approximately 7-10 business days after you complete this form and will reflect the exact details you submit here.

You can make updates to your profile after it appears in the ABDSM directory by logging into ABDSM.org with the username and password issued to you by the ABDSM.

The American Academy of Dental Sleep Medicine recognizes its members who are ABDSM Diplomates or International Certificants. Please allow an additional two weeks for your AADSM profile to be updated with your new status.

Current ABDSM Diplomates in good standing may use this symbol. ABDSM Diplomates are permitted to use the Diplomate logo with permission from the ABDSM and must agree to the terms and conditions for use. Diplomates and International Certificants of the ABDSM are prohibited from using the ABDSM organizational color logo in any manner. This includes websites, marketing materials, stationary, patient education materials, product literature, business cards, internal correspondence, promotional items, etc.
